I have a issue with my graphic card geforce 210

I have a issue with my graphic card only during game play.

I've got my pc 4 months ago.when i trying to play the ghost recon 2 advanced warfighter the connection of monitor and cpu is crashing while speakers giving some crashing sound,then monitor appears no signal and turn off.but cpu working still,only i can do that time is turn off by the CPU button,so that harms the system too.

After that I download the latest nvidia drivers 301.42 and installed it,then again I run the EA sports FIFA 12 game and above game.The problem is fixed for some days but again i experienced that matter.I replace driver with installing it again and again,But the problem is temporary solved and again happens.

if anyone can please help me to fix it.
