Assembly fan running like jet engine

When I first start up the computer the fan starts roaring and goes really fat like a jet engine. The only way is to press/hold the start button to get it to stop. Windows will not load during this. Computer tech said I needed new hard drive. Bought one and installed everything-still does it. I bought a new assembly fan and shroud (that is the fan that is going wild) and it worked for 2 days then it started doing the same thing. Any suggestions on what to do next?

According to the manual to my computer it is the assembly fan #T4307 Fan and Shroud It is covered with a green shield.
It is a Dell Dimension 8400 P4, 630 (3GHz), HT, 2MB, EM64T
1GB DDR2 SDRAM
250GB SERIAL ATA, 7200
