Windows hanging at starting windows

I recently built a pc, and it ran great for about 2 weeks. However, upon booting it up yesterday I got a hanging issue on starting windows. After raging a bit, and messing with everything I narrowed it down to the graphics card. My card is a evga gtx 670. I can boot my pc up using the onboard gpu. I tried uninstalling the nvidia drivers/reinstalling, but nvidia doesn't detect the card. The card is receiving power. I have two 6+2 pins in, only using the 6 pin part powering the card. I have a 1000w power supply thats reads a stable 11.96. I'm currently at work so I can't remember my other parts. Any help is greatly appreciated.

I have tried alternative connections however the only one that ever worked was the 2 6+2 pin connectors. Which only worked for the first two weeks. I don't have any friends with a power supply capable of running the card, so I'll prolly rma it in. Just wanted to check before I went that route.
