Looking for 4 port USB 3.0 for pci-slot to header

Does anyone know where I can find a 4 port USB 3.0 PCI Slot Adapter that will connect to 2 motherboard headers? Just like the one in the link below with but it would have 4 ports and 2 cables.

Yes, I would prefer it to be through the actual USB 3.0 headers on the motherboard.
