Display failing

CPU: AMD Phenom II X4 955
MOBO: AsRock 970 Extreme3
RAM: 8gig (4x2) Ripjaws 1333
PSU: Corsar TX 750W
GPU: Sapphire 6950 2gig
HDD: 500g Hitachi 7200

Problem: While playing games my display occasionally freezes and takes about 5 seconds until it minimizes my game and gives me the display drivers have failed error and its all back to normal.

Let me start out by saying I have posted about this on tom's before but I've recently done some things and none of them have fixed it. It is not a heat issue...I'm using MSI afterburner and monitoring my temperatures actively and they never go above about 53c.
I have tried EVERYTHING. I upgraded my PSU to the current tx 750 i have and that didnt fix it. I RMA my video card and that didnt fix it. I reinstalled windows and that didnt fix it. I've tried completely removing my drivers with driver sweeper and trying an older version and that didnt fix it. What in the actual *** could it be then? Maybe something as small as a problem with my PCI-E slot? i have no idea. Any input would be awesome...

ALSO I do have 2 monitors IDK if that could be causing problems or not..
