USAGE FROM MOST TO LEAST IMPORTANT: 4 monitors output from a single card(using mATX form factor), gaming on a single 1920x1200(Skyrim, Diablo 3, Starcraft 2, etc.)
CURRENT GPU AND POWER SUPPLY: EVGA GTS 450 and PNY GT 210 with Corsair AX650
OTHER RELEVANT SYSTEM SPECS: AMD FX4100 with 8GB RAM but might upgrade to intel down the road

PARTS PREFERENCES: Have had some driver issues with AMD 5770 a couple years ago and went Nvidia and had zero issues since.

SLI OR CROSSFIRE: Maybe in the future some time, nothing greater than 2 way
MONITOR RESOLUTION: 2 1080x1920 Dell Ultrasharp U2311H in portrait, 1 1920x1200 Dell Ultrasharp U2412 in landscape and 1 1920x1080 Vizio tv in landscape
ADDITIONAL COMMENTS: I want a single card that can run all 4 of my monitors, but still get decent gaming on my main 1920x1200 monitor. I want a very quiet GPU, as the rest of my system is pretty much silent.
