What causes DOA hardware?

Okay, so a lot of the time on newegg and review sites there is always large quantities of hardware that people say was Dead on arrival. Would it be because of human error ie; frying hardware from static etc? Or is the hardware just genuinely dead from the manufacturers? Can anyone elaborate on this more?

It could be user error, it could be poor quality control, it could be a bad batch of components, a shipping problem on the way to the seller, the seller mishandling the product (throwing unprotected harddrives in the bottom of the box with no padding).

No way to tell and it doesn't really matter. DOA is DOA unless the user.is an idiot and doesn't know how to use it. And you can spot those guys right away
 
Also, you will find a lot of bad reports on there because folks are quick to complain about something, but rarely post saying, 'nice job, fast delivery Happy as a clam'
over time it starts looking like an item is crap when its just the ratio of complaints to happy customers is skewed
more research on your chosen item is not only required, but well advised in any case
