3 monitors not working, worked before

I am trying to get my 5th monitor to work.

I know it says 3 monitors but technically it is my 5th.

I have two cards, gtx 570 and hd 5770
The gtx 570 has two monitors plugged

now the problem

before I bought my gtx 570, i had two hd 5770 with 3 monitors
it worked perfectly, did eyefinity and had them as extended desktops.
HDMI to HMDI
DVI to DVI
DP -> DVI ->HDMI adapter SAPPHIRE Active DisplayPort Adapter 100924 DisplayPort to DVI Interface

so now i have 5 monitors cause well i just do

same exact setup but with a gtx 570 with two extra monitors

the hd 5770 has the same plugs and adapter but now it won't turn on a third monitor

I can turn on any two at once but when ever i try to turn on the third it asks me to disable another one.
the monitors are different, one is a tv and the other is a tiny monitor from dell (16in) but i feel that shouldn't matter.

I have been looking for an answer everywhere for the past several hours

I have tried different combinations of hooking things up. i disabled the gtx570 to see if that worked. no success

not looking for eyefinity; Just trying to use it for extended desktop (tv for netflix)

Possibly a driver conflict.

Driver conflict: Clean out the driver and make sure it's installed properly. Then install the proper drivers.
