Sapphire rebate

I am trying to process a $20 rebate for Sapphire 7950 and one of the instructions says :
Original UPC bar code label

I got this card from new egg, so on the box i can find only this label

Is this the label they are asking for?

see on box itself that came with you sapphire card you will find upc number from sapphire, the new egg box is a packaging one
