First off i'll list my system specs so you guys get an idea of what i got.

i7 3770k 4.2GHz
MSI GTX 680 lightning 1375MHz/3550MHz (7.1Gbps) (i have another question about voltage)

Alrighty. when playing games like ACIII, Mech Warrior Online, World of Warplanes, especially GTA IV (although we all know it's one of the worst PC ports to ever exist) and some other games i get micro stuttering and tearing. although when playing games like BF3 and Black Ops II it's like butter. super smooth. i have adaptive V-Sync on and i tried just normal V-Sync to see if it helped, but it's even worse. Disabling V-Sync didn't do anything either. i disabled hyper-threading too and this didn't do anything but lower my temps by 2C then i tried resetting my GPU back to it's stock settings, but again it didn't do anything but lower my temps. why is stuttering and tearing only happening to me in some games? is it just the games themselves causing this?
as for my other question, i have a gtx 680 lightning and i flashed the BIOS back to the old version without the restrictions Nvidia added. i have my core, memory, and AUX voltage all set to the max so i could achieve the 40% OC i have now. because i have adaptive V-Sync enabled, it automatically changes the clock speeds and voltage in order to maintain 60FPS in games that are optimized well. in BF3 i'm usually hovering around 800-1200MHz but it does go up to 1375MHz sometimes and the voltage reaches 1.3V very rarely (usually around 1.2V) to keep that 60FPS. my temps haven't been higher than 65c (usually 55-60c) with the fans at 55%, but is the high voltage bad for my GPU anyway? when i run extreme burn in tests using furmark, of course my temps reach 90c with the fans at max, but it's still stable. i know this is very bad for my GPU. but besides that, is it bad for my GPU to occasionally bump up to 1375MHz and 1.3V even with temps below 65C?
and while i'm here, 1 more SMALL one i promise i get a very annoying buzzing noise through my headphones when my GPU is under any kind of load. i understand this is normal and the only way i can avoid it is by lowering the volume integrated into my Turtle beaches X12s to very low and then turning up the volume in windows 7. even after that it's still noticeable. now, would a PCIe soundcard get rid of this problem and possibly increase my sound quality? if so, any cheap recommendation would be appreciated

EDIT: i forgot to mention my slightly defective RAM. i can only get my system to POST when it is in single channel mode. (slots 1+2) anything else doesn't work on 3 different systems. i get blue screens around once a month because of it too. i've been meaning to RMA those for about 3 months i just haven't got to it yet. I highly doubt it has anything to do with stuttering and tearing, but i thought it was worth mentioning just in case.
