What would my problem be?

Hello,
Okay so I bought an evga gtx 660ti sc. I had to send it in for an rma bout a month back. Recieved a new one from evga. Sent it in for another rma this week. What would be my problem? Motherboard? Memory? PSU? The gpu crashes at simple games that don't require much.
I bought my psu open box. It is an Corsair ax 850. How would I test it if its broken?
Tried upgrading drivers to latest one, downgrading etc. Definitely not drivers.

Specs:
i5-3570k
ASROCK Z77 Extreme4
Corsair Vengeance 4 x 4gb ddr3 1600
Corsair AX850

It could be a faulty batch of cards on EVGA's side. Happened to me with Gigabyte once... You could just wait about a month and then order a new card so that it's from another shipment. I don't see anything wrong with your rig, it should be able to power the GPU without any problems.

Also try this to make sure virus does not cause this. fullscan system with "superantispyware"freeware.
Update windows .Automatically detect and install latest best driver and fix all issues.

Install old directx version and any plugins wanted to play u r game.
