AMD Radeon HD 5450 and dual VGA output?

I have a small form factor desktop PC which uses AMD Radeon HD 5450 graphics card. This graphics card has only one physical socket:

One should connect following Y-cable to this socket:

Am I correct that actually this graphics card has two VGA outputs, but in order to be as compact as possible, this card has only one physical socket and VGA outputs are separated by the cable?

Looks like this is a DMS-59 connector. Are there graphics cards which provide both 2x VGA and 2x DVI at the same time? In other words one could choose if he uses VGA or DVI connectors on the monitor simply by changing the Y cable. Or are the graphics cards with DMS-59 connector strictly either VGA or DVI cards?
