Ati Radeon Hd 4300/4500 What is it really?

I want to know what kind of gpu i exactly have. It says ATI Radeon HD 4300/4500. I dont want to open up my computer as I do not have the experience and will probably damage something. So what card do I actually have? Is it a 4500 or a 4300?
