Radeon 6870 strange problem

I don't really know how to explain this, so I recorded one of the times it happened. Normally, my games will run just fine for quite a while before this happens, took a good 6 hours probably before this happened today, been happening since yesterday. Once it does its thing it does in the video, the audio goes haywire simultaneously, as heard in the video about half way through. After that, I open task manager and end the process, it goes back to desktop where it notifies me that firefox has crashed, submit error and such. I try to restart and it immediately goes back to error. Some of the other things are missing as well, like when I try to exit out of steam, the bars at the top for minimize and close are gone, often the box with friends list is just a blank background, etc. I was not able to find any answers elsewhere online, so I hope that somebody hear can give me answers, although I suspect that my card is about to go out. http://youtu.be/MfQxQ32z2m0

Setup:
Gigabyte 990xa-ud3 mobo
sapphire radeon 6870 running on stock clocking now, had it up 60 past stock on gpu and mem. Still get error on stock.
amd phenom 2 1100t Oced 3.9 ghz
4x 4(i think, might be 8)GB G.Skill Ripjaw memory
soundblaster audio card, can't remember specifics.

It looks like something running in the background is trying take over the display. First clean your system with CCleaner. http://www.piriform.com/ccleaner
Do the Clean and Registry both.

Try again with nothing else running except the game/Steam. That also means disabling stuff you don't need running at startup. How many items do you have checked to run in System Configuration/Startup?

Another thing to try if the above doesn't solve the problem, is to uninstall the gfx driver again. Boot to Safe Mode and run Driver Sweeper. Have it remove any AMD, ATI, Nvidia graphic driver remnants it finds in the registry. Reboot and install the latest driver and profiles for your card.

I have already tried all of that, nothing in start up interferes nor do the drivers after a fresh install. A similar problem arises out of games in relation to this, when I run wprime95 after adjusting clocking by even 1 mhz, it will get black and white bars flashing on the screen at random, also took a video and uploaded it here.

Well, if you've been O/Cing your gfx card it may not have survived the bump you gave it. If you are sure the other possibilities I mentioned are not the case, that would be the next most obvious culprit.

I had suspected that this was the issue, and already have my replacement card ordered and shipping Monday morning, just wanted to make absolutely sure I hadn't overlooked any possibilities. Thank you for your assistance.
