Voice Commands not working

My T610 is ignoring my voice commands.

Firmware is R6B004, and I have about 15 Voice Commands associated to numbers
in the phone memory. When I use Voice Activation either through the phone
itself or through my Jabra BT200, the voice command is recognised and played
back to me, and then.... NOTHING! The phone doesn't dial, it just goes back
to standby.

Of course this is a real pain in the ass when I'm trying to call someone
whilst I'm driving as it negates the entire idea of hands free.

Whats wrong with my phone?

I had another T610 (R4Cxx) and a Z600 (latest firmware) which both did this
as well.

It recognises some, but not all.
