Heres the symptoms...
When it first booted, it says:
'A disk read error occurred
Press Ctrl+Alt+Del to restart'

OK, so I did that, I did ctrl+alt+del, it reboots, then I get to the menu in boot to pick safe mode, normal mode, etc. I pick normal, it reboots and gives me the read error message again, I choose safe and it gives me the first line showing the file name and path, then just stops cold.

I think the hard drive may be dead, any suggestions?

Sounds like a dying drive to me. You've made backups, right? If not, stop using the drive, get a new one, then copy important data from the old drive before it's completely gone.
