Model of the modem

Try to identify the model of the modem.
Updated from 98SE to 2K Pro.
2K Pro doesn't regconize the modem by itself.
Without knowing the model of the modem, unable to select the built-in driver, so many of them.
The information of the modem is:
Manufactured by Diamond.
On the sticker, p/n 23650001-001
On the circuit board, 22680001-002
fcc reg.# 4v4usa-27387-m5-e
Chipset: Conexant rs56-pci r6793-16 eg5619.1.

Yes, i did search the web without any success.
Any assistance would be appreciated.

You modem's FCC ID, is the first one on that txt file.

Thanks for the replies so far.
At this moment, i try to identify the model of the modem so i can select it in the Windows2000 built-in list.
I will review the fcc-id list, I think you lead me to the answer there.
Anymore assistance will be appreciated.
 
That second link wasn't the modem driver? I am thinking that second link is the correct driver.

It is either SupraMax 56K or SupraSST 56I
