The tool bar has disappeared

You left out an important piece of information - what application are you referring to?
My guess is you're referring to the Internet Explorer web-browser.

To get the menu bar back (it's a menu-bar, not a toolbar), right-click above the address-bar, then left-click "Menu bar" at the top of the context menu.
