How secure is Windows XP file encryption? Can a file be unencrypted or
accessed if it is removed from the machine where it was encrypted? Is it
worth doing for highly sensitive files?

Only you can answer if it is worth doing.
No one else is familiar with your situation.

EFS is very secure if properly used.
If a file is removed and the encryption keys are not available, the data is
safe.
If improperly used, there is a good chance you will permanently lose your
own data.
