Random monitor blackout, GPU overheating?

Hi,

I've been experiencing some random blackouts on my PC this week.
They seem to happen regardless of what I'm doing..
The screen shuts down for 3 seconds, and then it turns back on.
I've had only one BSOD during a lot of blackouts..
They occur randomly, like I said. While I was writing this 2 occured in 2 mins and after that none.
Sometimes the picture gets blue, my desktop, browser, whatever I'm running at the moment gets blue and slides down until it comes back to the place it was before. I'm sorry for the grammar and weird explanations, English is not my naitive language so I can't explain stuff that easy and precise.

My PC is:

Motherboard: Asus Croshair IV Formula
CPU: AMD Phenom 1100t Black Edition
GPU: ATI Radeon HD6850 Toxic
Case: Thermaltake v9 Black Edition
Monitor: Samsung SyncMaster 181t, but I'm getting a new one next week, probably SAMSUNG LS24B300HL/EN.
OS: Windows 7 - Professional x64
Memory: Kingston HyperX 1600MHz - 4GB - DDR3 Dual Channel

I've connected my monitor to my GPU with a HDMI cable through a DVI/HDMI converter on my monitor.
My CPU is clocked on 3.7GHz with stock fan, so maybe overheating could be a problem? CPU temp. is around 50*C on standard fan settings.

I have tried installing the new 12.6 Catalyst Control Center but it always finished with an error while installing the display drivers.
So I went to safe mode, ran DriverSweeper and deleted all of the ATI display drivers, and then installed them normally and it worked.
But now when i run the AMD Vision Engine Control Center (I think that is the new name for CCC) it says:

A lot of problems...

I am thinking about doing a full format of my hard drive and then installing a fresh OS..

Also I would like to ask, how can I return my CPU overclocking settings to stock/default.?
I have tried updating the BIOS, hoping it will return everything to default, but no succes..
And I have set the multiplier in ASUS TurboV Evo from 16.5 to 14.5 so now my cores are running @3262Mhz.

My CPU settings are:

CPU Frequency: 225MHz
CPU Voltage: 1.325V
CPU/NB Voltage: 1.25V
DRAM Voltage: 1.65V

I have never manually OC'd my CPU, TurboV Evo automatically overclocked it to 3.7GHz.

Well, in the meantime I've bought a new monitor (Acer s240HL) and sold the old one.
Since then I've had no problems with it, no blackouts, and the guy that uses my old monitor hasn't got any problems with it too...

Who knows? Try to clean your GPU fan if you haven't ever cleaned it.. Maybe it's overheating?
