Do I really need to uninstall and reinstall windows on my computer to get rid of spyware and malware? I'm not sure I'm selecting the correct categories.

Not necessarily, but you need expert advice on how to eradicate the infections otherwise you will get nowhere fast, and with bad advice you could even make the situation worse.
I don't see a dedicated malware forum on Tom's Hardware so you have no way of knowing if any advice you get on malware removal here is coming from someone who knows how to deal with it.
