I think the summary says enough:

Luthia the Ruinous (level 1, -2/8 HPs)
Began as a Deep Elf Conjurer on Aug 30, 2005.
Killed from afar by a smoke demon
... with a sticky flame (8 damage)
... in the Abyss.
The game lasted 00:01:42 (158 turns).

So yeah. Killed a goblin, rested up to max MP. She took a few steps, and
a kobold entered line of sight. Two magic darts, and down 'e went. He
dropped a "glowing dagger", though. Hmmmmm... interesting. She picked up
the glowing dagger, and wielded it. Uh-oh! A dagger of distortion! It
gave her that glow, you know, that says she's going to mutate pretty
soon. Better unwield it at once, before it does any more havoc! (<--
that's the silly bit of the death. Should have waited with unwielding
it.) Pressing ' to switch to slot a --- and we're in the Abyss. Okay. No
need to panic. Just run and hope for stairs.

No such luck though.
