Tornaments in D&D?

During a session tonight an interesting problem came up:

How on earth (or any other world) does a knight's jousting happen in
D&D-mechanics?

Picture this: two persons on horses, with lances, ride/charge each
other. One gets the initiative. Does both get charge bonus? Does either
provoce an attack of opportunity? If the clash - hits - are resolved as
at being done at the same time - which one gets the "first" hit?

This is addressed in the book Complete Warrior. IIRC, you roll
initiative each pass. Winner gets the first strike. You do not get a
charge bonus. If you hit, you deal damage. First one knocked
unconcious falls off of their horse and the other wins.
