I have just downloaded my Critical Updates and this was one of the
downloads. Does this work in the background or do you have to activate
it. I already have a AV which is updated daily so wasn't sure whether
this Update with interfere with anything and whether it should be
removed if that was possible.
I also noticed that when looking in the Add/Remove there was some
patches relating to SP2 which I haven't got on my machine yet, have
ordered the disc. Was wondering why they were downloaded being as I
haven't got SP2.

Lynn;
The tool does not install.
It runs, does its job and then is gone so there is no entry in Add/Remove
Programs.
More information here:

FAQ #2 & #3

The patches labelled SP2 are those integrated in SP-2.
Once SP-2 is installed, those entries will go away since they are part of
SP-2.
They were installed from Windows Update most before SP-2 existed.

So I presume that it has now run and then disappeared. I presume then
everything must have been okay or I would have had some sort of
notification. I presume then also that it is an item that will
continually appear in the Updates section then?

Lynn;
If you saw nothing, everything is OK.
There is a log, C:\Windows\debug\mrt.log.

Once you have run it for the month, Windows Update will not offer it again
until it is updated the next month.
Microsoft adds/changes monthly the specific item this tool looks for base on
know threats at the time.
This tool does not replace any other tools you may use.

See the link with the FAQ in my last post for details.

Is there any particular reason you didn't read the explanation
offered by the Windows Update site?

If that wasn't enough of an explanation for you, you need to read
the KB Article that describes the use of this tool. The MRT is updated
the second Tuesday of each month, appears as a "Critical Update" on the
Windows Update site, and runs only when and as "downloaded." Nothing is
actually installed on your computer; there's nothing to run at a later
date; there's nothing to ever uninstall.
