If I play a Wrench Mind

BB target player discards 2 cards or an artifact card from his or her
hand Socery

and have a Megrim down

2B Whenever an opponent discards a card from his or her hand, Megrim
deals two damage to that player Enchantment

does Megrim deal 2 damage to a player, they only discard once, or four
damage, they discard two cards?

4. They discard a card, twice, so Megrim triggers twice.

Short answer - Megrim deals 2 damage per each discarded card.

Long answer - the following rule:
410.6. An ability triggers only once each time its trigger event occurs.
However, it can trigger repeatedly if one event contains multiple
occurrences. See also rule 410.9.

Example: A permanent has an ability whose trigger condition reads,
"Whenever a land is put into a graveyard from play, . . . ." If someone
plays a spell that destroys all lands, the ability will trigger once for
each land put into the graveyard during the spell's resolution.

Megrim deals 2 damage for each card discarded. So, if they discard
one artifact, they take 2 damage. If they discard 2 cards, they take
4 damage, 2 for each card.

There is a small distinction to be made though. They do not take
"four damage from Megrim." They take "2 damage from Megrim" and then
"2 damage from Megrim", because the triggered ability triggers twice,
once for each card. This isn't a big deal unless there's some sort of
prevention in play. For example, if they have Circle of Protection:
Black, they will need to trigger it twice to prevent both hits from
the Megrim.

Wrench Mind BB Sorcery
Target player discards two cards from his or her hand unless he or she
discards an artifact card from his or her hand.

Slightly different from what you wrote there. (The player first decides
whether to discard an artifact card. If they don't discard one, they're then
told to discard two cards.)

Depends. Did they discard one artifact card? If so, Megrim triggers once,
and they'll take 2 damage when Megrim's ability resolves. Did they not
discard an artifact card, and discard two cards? If so, Megrim triggers
twice, and they'll take 2 damage twice, total 4 in the end, when Megrim's
two triggered abilities resolve. Did they not discard an artifact card, but
only had one card in hand, so only discarded one card when told they had to
discard two? If so, Megrim triggers once and will deal them two damage. Or,
did they have an empty hand and not discard anything? If so, Megrim doesn't
trigger at all.

If they discard two cards, Megrim triggers twice, yes. It doesn't say "Whenever
a player discards one or more cards, ..." - that would trigger once no matter
how many cards got discarded at once by the player.

"to activate it twice". CoPs have no triggered ability...
