Very probably. An object put into play by Sneak Attack is not
considered "played."

Put Into Play
If an effect instructs a player to put an object into play, that object
is not considered "played."

Well, you can't _play_ Phage via Sneak Attack; Sneak Attack doesn't let you
cast a creature spell in any way. You can _put Phage into play_ with Sneak
Attack, which is way different, and which doesn't satisfy "if you played her
from your hand", because you didn't PLAY her. So if you get Phage into play
via Sneak Attack (or via ANY other way that doesn't involve actually casting
her as a creature spell, and doing so from your hand as well), her ability
triggers and you're going to lose, yes. Unless you have a Stifle handy.

Yes, you lose the game. It did come from your hand, but it wasn't
"played"; it was "put into play". There's a difference.
