9600xt blocks

Dear Gurus, I'm here looking for clues to the problem with my Sapphire Radeon 9600XT 256MB. This is pretty lengthy, my effort to provide all the info up front to cut down on guesswork and back and forths.

PROBLEM SYMPTOMS:
Generally all 3D images are shot full of blocks and lines, including games and the Catalyst 3D Preview racing car demo. [Screenshots available on request].

Except for a single 3 hour period a week ago, this 5 month old vidcard has never worked properly. For a couple of months I was able to play Unreal Tournament 2004 with all settings turned way down or off at crappy resolution and terribly slow framerate. A recent ATI Catalyst driver upgrade finished that off. Blocks full time now.

Usual 2D graphics are fine until a 3D app is started, then a yellow horizontal bleed is seen on light colored backgrounds (documents, windows, etc). Blue bleed/trails on dark backgrounds. Grabbing and shaking a window has greatest bleed effect. Bleed is only rectified by reboot...until next 3D app is run (including the 3D Preview car demo)

Sometimes (only after a reboot) the Catalyst 3D preview car is smooth and the card appears to be functioning properly, but this lasts perhaps a second before the blocks appear, followed by the screen bleed.

3DMark also showed a short lag effect before lapsing into serious blocks/lines to the point where I couldn't even guess what the screen was meant to be showing. Bleeds started.


FIXES TRIED:
I have applied new drivers wherever I can, even used "Driver Cleaner 3". Last week I reinstalled the ATI drivers this way and turned off Fastwrite and as mentioned above it was the only time the card worked - HalfLife2 and UT2004 worked beautifully until the system was rebooted. Never before, never again, despite repeating the re-installation several times.

I have fiddled with Bios and ATI Catalyst settings with no improvement.


CURRENT OPINIONS:
This time-lag symptom leads me to wonder if a setting is automatically being changed after the 3D app starts, perhaps as a buffer fills up...??
Others have speculated that there might be a bad connection on the Vidcard itself...?
If this is a mobo/vidcard incompatibility then how come it worked for 3 hours?

PLEASE HELP!!!
I have transcribed my complete BIOS and ATI Catalyst settings, available in TXT format, for anyone who may be able to point out an incorrect settings. I know, I know, some of these settings have no bearing on the problem, but I don't know where the boundaries are so I've included ALL of them :-)

Ever try to run DXDIAG?

Click Start ---> Run and Type "DXDiag" without the quotes.

Click the display tab. You should see:
Direct Draw acceleration : enabled
Direct 3D acceleration : enabled
AGP Texture acceleration : enabled

Try clicking both Test Direct draw, and Test Direct 3D, and see if you pass them.

Thanks for the tip, I just tried them and they all work fine. Does this narrow the possibilities?
