If limited to desktop and start menu items, try Control Panel (classic
view)> Taskbar and Start Menu properties. Click Start Menu tab to bring it
to the forefront. Click Customize. Place a check next to "enable drag and
drop." OK your way back to desktop. Now try a right click.

If still stuck and you have XP Pro, check settings in gpedit.msc and in
secpol.msc that relate to access of right click menus.
