Is there a way to add additional sockets to a socketed item? I found a
3-socket Monarch, and I was wondering if it could be made 4 sockets for
another Spirit shield.

Your best bet here is to run around with 0% MF in places like the
Ancient Tunnels (act 2) of Hell. If you get a monarch to drop then ask
Larzuk nicely to socket it for you. He always adds the maximum sockets
to normal (non magic) items.

Other level 85 areas: The Mausoleum under Blood Raven, and The Pit in
the Tamoe Highlands (mausoleum is easier to get to).

No, it's 83. Crypt and Mausoleum are 83 and 85. Can never remember
which is which.
Btw, a lot of people do cow runs without MF to get their white monarchs
I think.

Yes.
If the item has a low ilvl, you will get the lower number of allowed
sockets.
e.g. if you find a damaged/cracked/low-quality Monarch, and repair it to
normal with the cube formula, the ilvl of that Monarch drops to 1, even
if it was 85 before. Larzuk will give that shield 3 sockets.
I don't know the ilvl at which Larzuk will give the Monarch shield 4
sockets, but (for the Monarch, and most other elites) there's a sweet
catch: any monster with high enough mlvl to drop a Monarch will drop one
that will get 4 sockets at Larzuks.
