Vsync, from what I understand, is a process which prevents screen tearing. A more detailed description about tearing and ways to prevent it can be found here, though know I just googled it...
When it's off, your performance increases, and when it's on, you won't get the tears. I can't actually run anything graphics intensive on my computer, so vsync is usually off.
