What should I upgrade on my rig to increase gaming performance?

I had my PC for 2 years and I think it's time I did an upgrade.
The question is which part should I upgrade first that will give me the most significant improvement in terms of game performance?
I played the latest Tomb Raider game at low settings, and as expected, the results were disappointing.

Here are my current specs:
Mobo: Asus M4N68T-M LE V2
CPU: AMD Phenom II x4 840 @3.2GHz
Graphics: Sapphire HD5450 1GB DDR3 64bit
RAM: 4GB @1333
HDD: Seagate 500GB 7200rpm

But before we can make any firm recommendations we'll need to know your monitor resolution and power supply details-Make, model and declared output, both total Wattage and Amps on the 12 Volt line or lines.

if you have a 64-bit OS, increase to 8GB (2x 4GB) of memory

having a Processor with L3 Cache might help like a Phenom II x4 965

and also a better video card
