The past hour, I am triggering this quite often. Ranges from opening a thread, replying to a thread, adding tags to threads, refreshing the forum/all.html, forum/unanswered.html pages, or the followed tags timeline page.

Reporting to the development team for investigation. It may have been a temporary glitch or server maintenance issue. Has it happened again for you since that time period?

I've encountered one in the last 12 hours. And I think only 2-3 since then so it has gone down dramatically.

I've seen it happen in the past numerous times, but it's highly sporadic and usually doesn't happen when resubmitting the request. I don't know any guaranteed steps to reproduce. It can pretty much happen anywhere (so probably a server problem).
