Kernel-Power 41 bluescreen & GPU Crashing

I've recently finished my computer build, and for the past week it's been giving me two issues, which, I want to assume it revolves around one, I've been getting blue screens with Windows 7, and I cannot figure out the problem, the only time it happens is when I'm playing Counter-Strike: Global Offensive.

There's two different scenarios to the crashing, for example:

1: While playing my game, my computer will take no time, go blank, then show a blue screen notifying me of a "Kernel-Power" shutdown, when I check in "Event Viewer", I currently see 9 instances of the problem.

2: While also playing my game, my screen will go black, and whatever louder sound was playing last, will stutter, my monitor will say "NO INPUT", and then it will either not load the picture again, or (so I'm assuming) my graphics card will restart and show the picture again.

I can provide additional information if necessary.

When all of this first started happening, I removed one of my additional HDDs.

I've also run an HDD test (passed), RAM test (passed - using Memtest86+), and CPU / GPU burn test (both passed).

RAM: G.SKILL Sniper 8GB (2 x 4GB) 240-Pin DDR3 SDRAM DDR3 1600 (PC3 12800) Desktop Memory Model F3-12800CL9D-8GBSR (Only 1 for 4GB)
Crucial Ballistix Sport 16GB (2 x 8GB) 240-Pin DDR3 SDRAM DDR3 1600 (PC3 12800) Desktop Memory Model BLS2KIT8G3D1609DS1S00 (Only 1 for 8GB)

CPU: Intel Core i5-3570K Ivy Bridge 3.4GHz (3.8GHz Turbo) LGA 1155 77W Quad-Core Desktop Processor Intel HD Graphics 4000 BX80637I53570K

Motherboard: ASRock Z77 Extreme4 LGA 1155 Intel Z77 HDMI SATA 6Gb/s USB 3.0 ATX Intel Motherboard

GPU: NVIDIA - GeForce GTX 660 Ti 2GB GDDR5 PCI Express 3.0 Graphics Card

Running Windows 7 Professional (SP1)

If there's anyone that can help me, it is much appreciated, I've tried updating drivers, reformatting, cleaning my computer of dust (brand new, no room for dust).

You got mixed ram with different ratios, you can't do that, it will cause memory management errors.. You can only use mixed ram module if your motherboard supports single channel ram slots and single channel ram.

Remove the Crucial Ballistix Sport 8gb module and replace with the other 4gb G.SKILL Sniper.
What ever were you thinking would happen using one of each ram type?

You do need a PSU that will provde adequate power for your rig.

I actually realized that, no idea why I mixed the two, however, that was not the issue, I have 2 G.SKILL Snipers in there now (both original package), and my PSU is 750 watts, still does it.

Have you stopped any Service?
Did you stop anything in the Task Manager?

Did you overclock the odd ram when you had it in the rig prior to the swapover? You need to set your ram to its default settings ( Save & Exit bios) before attempting to O/C them again.

What's attached to your usb ports?
