Hardware upgrade - Help needed

I built my gaming PC in 2011 and hasn't been upgraded since then. Now that I've saved some money to put into it, I'd like to consider various upgrade options and go with it.
I've got down my current PC configuration. Please look at it and advise what are the components that can be upgraded for a noticeable difference in performance.
Performance can be everything from FPS, number crunching speed or faster load times etc...

In other words, If you were me, what would you upgrade on this rig? Bear in mind that the budget is around $500

Mobo - ASUS M4A88T-V EVO USB3
PSU - Corsair TX650W
Proc- AMD Phenom II X4 965 Black Edition 3.4Ghz
Heatsink - CoolerMaster Hyper 212 Plus
Graphics - XFX Radeon HD 6950 2GB
RAM - 8GB@1600Mhz DDR3 Gskill
HDD - 1TB SATA2 from WD
Case - NZXT M59 001BK

I would add an SSD for fast load times, and maybe upgrade your GPU, although yours is still pretty decent.
