Is my disk failing?

Windows gave me a message that it detected a hard disk problem. On restart SMART issued a message to backup and replace.

I ran HDDScan.

1. SMART Report

2. SMART Conveyance Test
I get this message and the test stops after 10%.
Self Test execution status: Fatal error with unknown element failed.

The CRC errors could be caused by a bad or loose sata cable. Try replacing it first and see if the error count increases or remains stable. You also have some reallocated sectors. I'd backup the drive and be prepared to replace it. Hopefully it's under warranty. Anyhow, try a new sata cable and possibly a different sata port and see if it fixes the problem.

Have multiple drives. I swapped power and data cable. The problem exists on the same drive.
I ran, Hard Disk Sentinel and I got this,

Failure Predicted - Attribute: 5 Reallocated Sectors Count, Count of sectors moved to the spare area. Indicate problem with the disk surface or the read/write heads.
There are 2709 bad sectors on the disk surface. The contents of these sectors were moved to the spare area.
Problems occurred between the communication of the disk and the host 11673 times.
Replace hard disk immediately.

I guess its only matter of time before this is toast?

Yeah, not a good sign. Backup what you need while you can. Manufacturer's are pretty good at replacing drives under warranty.
