Is my disk failing?

Windows gave me a message that it detected a hard disk problem. On restart SMART issued a message to backup and replace.

I ran HDDScan.

1. SMART Report

2. SMART Conveyance Test
I get this message and the test stops after 10%.
Self Test execution status: Fatal error with unknown element failed.

Have multiple drives. I swapped power and data cable. The problem exists on the same drive.
I ran, Hard Disk Sentinel and I got this,

Failure Predicted - Attribute: 5 Reallocated Sectors Count, Count of sectors moved to the spare area. Indicate problem with the disk surface or the read/write heads.
There are 2709 bad sectors on the disk surface. The contents of these sectors were moved to the spare area.
Problems occurred between the communication of the disk and the host 11673 times.
Replace hard disk immediately.

I guess its only matter of time before this is toast?
