Dell Inspiron 620 Graphic Card Upgrade Recommendation

I know this question has been asked a lot but I'd like someone to recommend me any recent graphics card.

Processor: Intel Core i5-2320 CPU @ 3.00 GHz
RAM: 4 GB

I'm willing to upgrade my PSU as well but I'm not even sure which one to buy as I am very much new to computer parts. My aim is to be able to play some upcoming games such as Dragon Age or perhaps Witcher 3 on medium setting. I've also searched around forums and found a lot of people recommending the GTX 750 ti but I'm not sure if it'll be powerful enough. I don't really have a budget right now but preferably something not too pricey. Also i've found it very hard to find one that is less than 10 inches long and will fit the inspiron 620.

With the stock Dell PSU, the GTX 750 Ti is the fastest card that doesn't need a 6 pin aux power connection. (some do, so be careful which ones you look at)

It is factory OC'd and will run any game at max/near max settings at 720p or with slightly reduced settings at 1080p.
