Driver keeps on crashing and recovering even on a small monitor

So last month I asked a similar question and it temporarily fixed the problem.
I have a small 19" VGA Monitor and it STILL keeps on crashing and recovering,
It crashes about 5 times an hour and im very scared that it will break my gpu..

I also have graphical glitches when in-game (textures spazzing out and stuff)

This is getting really annoying, and I asked the guys over where I bought the GPU and they said to reformat my PC, I havent done that yet cause I have no time at all.

Any other solutions? I will try to reformat.

Could you please list all your components cpu, motherboard, gpu, etc.
