Third monitor not working

Presently, I have two DVI monitors hooked up to my computer and I'm trying to hook up a third, VGA monitor. Everything is hooked up to my PCI-E graphics card with the VGA monitor hooked up with a VGA/HDMI adapter. The problem-- it's not showing anything except "Power Save Mode" on the monitor. I checked the Power Settings and I don't think anything has timed out. I tried updating my graphic's driver and all three monitors worked at first but then, next time I fired up my computer, it wasn't showing anything. Any ideas on how I can fix this problem and get all three monitors up and running?
 
Solution
I believe you can only use 2 dvi or 1 dvi and hdmi for 2 monitors and then you have to use the display port for the third. make sure you get and active display port cable if you need to buy one.

Hmm I'm not sure. Quite an unusual combination of monitors and connections. I don't think I've actually ever heard of or seen a VGA to HDMI adapter. Very strange indeed. Most 3 (or more) monitor setups use at least one mini displayport or display port alongside two DVI connections or one DVI and one HDMI.
