That sounds like your card is more powerful than needed and for that game you need to tame the cards frame rates by enabling v-sync. This will force the GPU to refresh with the rate of the panel and hopefully reduces tearing. If tearing continues then you might need to turn down the graphics settings.

Lines is usually a sign of a graphics card that is overclocked but under volted. Try raising the volts on your graphics all the wile watching the temps. Don't raise them too high or your temps will sky rocket. You could also try under clocking the card.

The other thing that could be causing this is the power supply. You might want to test the card in another computer to see if the problem is in the card or the rig.

Maybe do this first before you start overvolting because if you overvolt on a bad PSU it can cause more problems.

Not so much now days, you may need to go in and increase the voltage to your GPU from your mobo. My brother's GPUs ( (x2)7850 in crossfire) were crashing due to his board not automatically giving enough power for them. Try that, otherwise its because that resolution is not a very friendly one. If you had one like 720p, or 1080p it would be for friendly to that GPU. Typically 1920x1080 is the best resolution for most GPUs now days. Anything less the GPU actually has to reduce its performance which could cause tears.
