Gigabyte GTX 970 Wind Force Not Working

Hello, I about a week ago I built my first computer ever, and it was fairly difficult, but I got it done, and everything worked great for about a week.

Yesterday, after about a 4-5 gaming session in Shadow of Mordor(Great Game) I was just browsing the internet while I was in a skype call when my monitor when black. I turned off my computer and when I booted back up it worked for about 15 minutes when I went black again. So I decided I just let it sit over night and see how things were in the morning.

Come this morning I turn on my computer and it boots on, but doesn't make it to the log in screen without going black again. I figured that it was port problem so I switched both hdmi ports on my monitor, as well as trying dvi, but neither would work.

The fans and the LED on my 970 both work and are on, but not port. I also tried switch PCI-E ports and that had no resolve.

The only way I could get anything output to my monitor was by plugging the hdmi cable into the hdmi port on my back I/O panel on my motherboard. I thought this had fixed things until I tried booting up Dead Rising 3(Another Great Game) and the loading screen took forever and the game lagged like crap, leading me to believe that it was running off integrated graphics on my motherboard itself.

I am sure what to do, hopefully one of you bright minds do.

Side Note: I bought the card from superbiiz.com (the only place who had one in stock) less than a month ago and I am unaware of replacement policy if it is less than 30 days old.
