Black screen on first monitor after plugging second monitor

I don't have this problem before since I used this graphic card ( GTX 970 FTW)for 3 days. The 4th day when I tried to organize my cable. I forgot to unplug the power. I switched the DVI cable and HDMI, VGA cable to test. But when I turned on the computer. There was a line say code error 0094f somethings. After the windows logo disappeared, all of my 2 monitors blank. I tried to unplug my graphic card and the second monitor. Then I plugged the graphic card back. It worked fine even a little bit slow. When I got into the main screen, I tried to plug the 2nd monitor back. And then, the 1st screen disappeared (blank). All my desktop screen moved to 2nd monitor. I removed that graphic and plugged the old GTX 550ti which I used before purchased the Gtx 970 ftw. Everything worked on both monitor. I tried to refresh windows 8 but it doesn't work. So is it motherboard problem, bios or graphic card? and how to fix it
My specs:
Intel i5 3350p 3.0ghz to 3.1ghz
The motherboard from dell: Dell inc. 0nw73c
Ram: 8gb 1600mhz
The GPU has problem: GTX 970 FTW 4gb
The Old GPU that worked fine: GTX 550Ti
Bios version A12 from Dell
PSU: EVGA 850W
I am currently using windows 8
