What are the ingredient that we need for triple monitor setup?

I am new that the one who wanna setup triple monitor setup. I wanna know what i need between Monitor and GPU. I gonna use EVGA GeForce GTX Titan Black 6GB 384-Bit and use monitor as LG branded television which has 40" and all i wanna know is what we need to setup between TV and my GPU. (Note: my LG television has only HDMI port)

You don't need much. You need all three screens connected. You may need some converters such as display port to hdmi if you plan on using the same screen 3 times. Also may sure to activate it in control panel. Nvidia control panel>3D settings>configure sli, physx,surround>span displays with surround.
