Upgrading system for 2015 games and some photo/video editing

I'm wanting to get my computer up to date so it will competently play modern games. I don't need to have them on the highest graphical setting, just so that they'll run smoothly. I may also do a little bit of photo and video editing.

Here is my current build. Does anything in particular need upgrading? If so, what would you recommend?

CPU: AMD Phenom II X4 955 (3.21 GHz)
Mobo: ASRock 880GM-LE FX
RAM: DDR3 4GB
GPU: ATI Radeon HD 3850 Super DDR2 512MB
PSU: Corsair HX 520W
SSD: Crucial M4 2.5" 128GB SATA III
OS: Windows 10
Case: ThermalTake LanBox VF1000BWS (micro-ATX)
Monitor: Samsung SyncMaster 933

Fallout 4, Dark Souls 3 when it comes out. Like I said, I don't need the games to be on top settings, just so long as they're is playable without any slowdown. Budget.... maybe up to US$150 for the graphics card (unless someone can convince me I need to go higher) and maybe up to US$100 for the motherboard?

Is your CPU throttling with that motherboard? By a new motherboard I was suggesting a platform upgrade which would require a new CPU as well. Such as an i5 and H97 motherboard, but it seems like that is out of your budget. A new GPU would make the largest difference to your pc as your current one is not suitable at all for modern gaming. Around $150 you should be able to afford a GTX 960 or R9 380.
