Computer constantly freezing for 1 minute - Grand Theft Auto 5 unplayable due to constant stuttering

As of late I have taken to monitoring my computers performance due to some reoccurring issues. I have had my computer (ASUS CG8270) for a while now, bone stock, and for a very long time it has had an odd freezing phenomenon. Once it happens, my C Drive shoots up to 100% usage, and oddly enough, it without a doubt lasts for almost a minute exactly every time. I'm not sure what is causing it, as none of my programs in the Task Manager Processes tab appear to be acting up at the time of freezing.

I am curious as to whether or not it could be linked to a different problem I am having, which is GTA 5 freezing for about 3 seconds, and unfreezing for about 1 second constantly. It is saved on my D drive, and for the longest time worked without flaw. My GTX660 had no problem with processing the game, but in the last few months does not seem to want to run it. However I suspect it is not my GPU to blame, it could be my HDD, Is it possible that the read speed is no longer fast enough to supply the information to my game? My TM Performance graph shows constant spiking for my D drive while the game is playing, and at this point I'm stumped. I have no problem playing any other games.
