BSOD GPU driver issue or hardware issue?

This recently happened, my monitor went black and I got a BSOD. I get screen pixelation and horizontal lines on my monitor. This occurs in safe mode too but not as extreme. I tried reinstalling/cleaning my HD 7970 gpu drivers, but nothing.

Is this a driver or hardware issue?

Update: After some playing around everything seems fine until I start doing something like opening Facebook, then the screen goes to black and I get error saying the driver display has stopped working but has recovered.

I assume this is a hardware error, probably a dodgy connection or insufficient power supply to the GPU. Recheck the connector pins and make sure they are plugged in flush to the PSU and GPU.
