Windows 10 Blue Screen ( PFN list corrupt )

Recently for the last couple of days I've been getting strange errors on my computer.

My computer is getting a bit old but it was custom built and I've never had any problems with it till now. No freezes or nothing.

First error was when I was watching videos on youtube and twitch streams. This got my screen flashing to black, everything frozen, and then it comes back after a second or so with the error. The Display driver AMD has stopped responding and successfully recovered. This happened repeatedly ( every 5 seconds ) until I closed the videos.

On that day when I started playing league of legends ( 10 minutes in game ). My entire computer froze, the sound repeating itself in a loop, for about 30 seconds before it came to the blue screen of death. I didnt get the error message that time.

Then today while I was playing league again I got a bug splat around 10 minutes in game again. Upon attempting to restart the client, my computer just froze and turned black no error message whatsoever. I restarted it and then about 2 hours later while I was playing league again, I got another error. This time it froze outright before flashing to the blue screen of death. The error message was different than the last time that I know, this one being a PFN List corrupt.

I'm not sure why I'm getting these messages but I really would like to believe that my computer is dieing.

Update: Just ran driver verifier and got a smartdefragdriver.sys error. uninstalling smart defrag now, is there something else I should do?

I suspect something gone wrong with the newest driver or windows update here, I got this around 28 april and randomly happen til today, I build a lightmap on unreal engine 4, then it crashed after it completed the build
