Edit: You don't even need 7 or 8 when doing an upgrade. You just need a valid key. The 7 or 8 key will work to activate 10. For another week at least.
If you are doing a free upgrade, then yes. You have to have a working win 7 or better OS. You have to run the installer from inside windows. (Free upgrades may be over...)

If you already have upgraded, activated and are reinstalling, or bought a new copy of windows 10. Then no. You can just install windows 10.

No you do not.
With the 10586 release back in November 2015, you can do a clean install of Win 10, and use your existing Win 7/8/8.1 license to activate when it asks for a license key.

To repeat...you do not need to install Win 7/8/8.1 first.
