Should I upgrade? If so, which?

Hello, I am looking to upgrade my CPU and/or GPU since its' been some time since the last one.

I aim to run on 1080P, Just want a smoother experience, but on a budget.

Currently I'm running:

MSI 785GM-P45 Motherboard
AMD Athlon II X4 635 2.9Ghz
Geforce GTX 550 Ti 1GB
12GB RAM

I've noticed new games now are running slow on Higher settings. I would like to be able to play on High settings again :(
(Game examples: FarCry4, LOTR: Shadow of Mordor, Planetside 2, Starcraft II)

Any Considerations for worthy bang for your buck replacements? I have checked the hierarchy guides here and I am looking for a personal opinion.

Depending on your budget and if whether you want to upgrade your CPU, I would personally get this setup:
NZXT S340 Case
Any Z170 motherboard
Intel i5 6600k
MX-4 or Artic Silver 5 Thermal Paste
Hyper 212 Evo, Cryorig H7 or any AIO liquid cooler.
GTX 1060 or AMD RX 480 (I prefer the 1060)
8 GB of DDR4 ram or more if wanted
EVGA GQ 650W power supply or Corsair RM750W

Now, I don't have a system as great as this but I bet that this will run really good on a lot of games. This is what I have:
Rosewill Tyrfing Case
Gigabyte GA-Z97X-UD3H
i5 4690k @4.0GHz
Artic Silver 5
Hyper 212 EVO
MSI GTX 960 2GB (2x Armor)
(2x4) G.Skill RAM @1600MHz
EVGA GQ 650W Gold+

My build isin't the best but it's not the worst. I'd get a reference cooler if you got my setup because it does get the case a bit hot sometimes but it will run pretty much everything on ultra and so will the system that I recommended.
 
^a 1060 or a 480 will smash that 960. They can run 1080p60 ultra in pretty much anything,the 960 cannot. It'll get pretty close or it will if will to lose fps.

Your suggested build is excellent. I went with i3 as it seemed op was on a tight budget.

I know, I only got the 960 because I found it for $160 brand new. This was like 4-5 months ago. And i'm trying to sell my 960 because I want a graphics card with a reference cooler, main reason being that it gets a bit hot in my case and gets the CPU heated up from the heat of the graphics card.
