Monitors occasionally go out of range for a few seconds when playing video games. Sometimes restarts PC.

All of a sudden, when playing video games, my monitors will go out of range for a few seconds before returning to normal randomly during play time. This doesn't happen all the time, but it is fairly common. It usually happens around 5minutes into playing but I've had it happen an hour into gameplay. This issue only happens ONCE after loading the game. If I close the game down and re-open it, the problem will happen again.
This doesn't happen when doing basic tasks such as internet browsing, just on video games.
Another issue associated with this issue is the computer restarting, however this is a rare occurrence.

GPU - GTX 970
MOBO - H87M PRO4
PSU - CX850M
RAM - 8GB
Monitor - BenQ GL2460HM

It may be problem with your refresh rate, have you tried following this guide?
