Monitor delayed/lag/some sort of mystery problem?

Ok so my problem is , my monitor seems to be odd, everytime something fast happens for example when im watching this quick color change or just some random videos or something fast paced that is happening . my monitor acts like this. tried going safe mode and the problem is gone, I solved it before after removing the rainmeter app, but now its back. tried formatting and it helped but not for long, just after a month its back again

are you sure that it is the monitor? have you tried plug another pc on it to test it?
